Instructor and Assessment Information

This form provides us with instructor contact information and information about the exam(s) that students will be taking. By indicating exam dates we can monitor the usage of the testing center to ensure that the room does not get overbooked. The form also provides us with what resources students can or cannot use during their exams.

IMPORTANT: Please provide the Testing Center Proctor with your course roster at least two days prior to the first exam.
